At the heart of our home health care are our Patient Advocate | Health Coach teams. These pairs oversee and coordinate our patients’ care. The Patient Advocates are highly skilled Registered Nurses. They assess individual patients’ needs, establish the plan of care, coordinate the necessary clinical disciplines (nursing, therapy and medical social work) and communicate with the physician. As a home health care agency, the members of the Care Delivery Team are the faces of THA Group whom our patients see every day. We have nurses dedicated to admissions and discharge. Vicki Bolen, RN WOCN, is a renowned Wound, Ostomy and Continence Nurse who oversees and visits patients with complex wounds.
Jon Cohen, RN CHPN, is our hospice and palliative care specialist who consults with the Patient Advocates and Care Delivery Team for pain and symptom management and other associated issues.
Dana Canestrari, RN, helps implement scheduling of the Care Delivery Team and ensures patients receive appropriate care in the most timely manner.
Vicki Mohr (VP), RN; Kathy Johnson (VP), RN; and Diana Shelley, RN, are the Performance Excellence/Education triumvirate who keep our staff on point with the up-to-the minute training and ensure that our staff is “the best of the best.”
Cathy Cribbs, RN, and Shembra Carter, RN, are our after-hours triage nurses. Unlike some organizations that provide an answering service, THA Group provides actual nurses who answer patients’ calls immediately.
Cathy Hess, RN, and Georgeann McDonald, RN, are experienced telehealth nurses who extend the reach of our Patient Advocate / Health Coach teams.
Finally, Ellen Bolch, our company’s President and CEO is a Masters prepared nurse whose experience runs the gamut from hands-on labor and delivery nurse to university nursing faculty, to business executive. Her more-than-thirty-year career gives her the wisdom and insight to lead our company into the future and guide our dedicated team to live out our mission: Health Care. For Life. At Home.
